Coroner Clare Keane returned a verdict of death by suicide at an inquest into the death of Neville Kearns at Dublin District Coroner’s Court on Tuesday after hearing evidence that the collision was due to the result of “driver input.”

The teenager’s parents broke down in court as the HSE apology was read out as they settled a High Court action over the death of their daughter four years ago.

The Samaritans called for urgent action to address the root causes, including poverty.

Det Supt Fox (56) – a married father of three from Seabury Glen in Malahide, Co Dublin – was the lead investigator into the fatal shooting of David Byrne at the Regency Hotel in Whitehall, Dublin on February 5th 2016.

These findings, from a survey conducted in 2021, are published in advance of World Mental Health Day on Tuesday.
